TO RECEIVE YOUR $50 REBATE CARD:
1. Mail the following items to the address below using a separate rebate form and envelope for each rebate request.
- The rebate form with all fields completed (download the form here).
- A photocopy of your sales receipt or order confirmation sheet for the qualifying item purchased. Item purchase date must be within 11/1/2014 and 1/15/2015 and be clearly visible. Please circle the line item on the receipt.
- The entire proof-of-purchase label (UPC label) from the device box. Cut or make a photocopy of the original label from the double pedal box.
2. Make and keep copies of all rebate submission materials for future reference.

- You should receive your rebate within 6-8 weeks after your rebate request is received and verified.
